General Directions

• Right click the registration file below and save to your hard drive.  The file is in Microsoft (c) Excel format (.xls).  One of the forms is a print-only form, the other one has tables set up for automatic calculation of entry fees.

• Be sure to complete ALL THREE pages.

    Option #1

• E-mail ALL THREE pages from the workbook to meet@prestongymnastics.com

• When sending your e-mail, be sure to include your club name and USAG Club # in the subject line.  The USAG Club # will be used to verify your athletes online.  Any athletes with missing information will NOT be entered.

• Final registration is not complete until a check has been received by Preston Gymnastics Academy.  Send check to: Mid-Atlantic Invitational, Preston Gymnastics Academy, 7967 Cessna Avenue, Gaithersburg, DM 20879.  If sending an early registration deposit, simply include a check for $100.00 made out to Preston Gymnastics Academy.  Balance due by regular registration deadline.

• You will receive a confirmation e-mail once registration has been processed.

• We do not accept registration forms other than our own!!

• Send the completed pages and payment (check only) to: Mid-Atlantic Invitational, Preston Gymnastics Academy, 7967 Cessna Avenue, Gaithersburg MD, 20879

• You will receive a confirmation e-mail once registration has been processed.
• No athlete with missing information will be processed.
